Fecal microbiota transplantation versus glucocorticoids for the induction of remission in mild to moderate ulcerative colitis

OBJECTIVE: To compare efficacy and safety of fecal microbiota transplantation (FMT) with glucocorticoid as induction therapy in ulcerative colitis (UC). METHODS: The patients with active mild to moderate UC were recruited into the single-center, prospective cohort study. The patients were treated with either FMT (FMT group) or glucocorticoids (GCs group). Patients received FMT administration for 3 days. The primary outcome was clinical and endoscopic remission at week 12. Inflammatory parameters were assessed by routine blood tests. Safety was assessed by adverse events recorded. The serum levels of TNF-α, IFN-γ, IL-1β, IL-4, IL-5, IL-6, IL-10 IL-8, IL-12p70, IL-13, IL-17A and IL-23 following FMT were measured by Luminex multiplex assay. RESULTS: Of the 122 patients, 62 patients were treated with FMT and 60 with glucocorticoids. 34 patients in FMT group (54.8%) and 29 in GCs group (48.3%) reached the primary outcome (p = 0.30). The incidence of adverse events in GCs group (35/60, 58.3%) was significantly higher than that in FMT group (14/62, 22.6%) and two serious adverse events were observed following GCs. Patients in FMT group were stratified into responders (RE) and non-responders (NR) groups. The level of TNF-α and IL-6 decreased significantly in RE group, while IL-10 decreased significantly in NR group. CONCLUSION: FMT therapy was as effective as glucocorticoids to induce remission in active mild to moderate UC, accompanied by fewer adverse events. The modification of serum TNF-α, IL-6 and IL-10 might be related to the efficacy of FMT in UC. Trial registration This study was registered with ClinicalTrials.gov (NCT02435160). Human embryos harbor complex mosaicism with broad presence of aneuploid cells during early development

Pre-implantation genetic testing for aneuploidy (PGT-A) is used in approximately half of in vitro fertilization cycles. Given the limited understanding of the genetics of human embryos, the current use of PGT-A is based on biologically uncertain assumptions and unvalidated guidelines, leading to the possibility of disposing of embryos with pregnancy potential. We isolated and sequenced all single cells (1133) from in vitro cultured 20 human blastocysts. We found that all blastocysts exhibited mosaicism with mitotic-induced aneuploid cells and showed an ~25% aneuploidy rate per embryo. Moreover, 70% (14/20) of blastocysts contained 'chromosome-complementary' cells, suggesting genetic mosaicism is underestimated in routine PGT-A. Additionally, the analysis of 20,945 single cells from day 8-14 embryos (in vitro cultured) and embryonic/fetal organs showed that 97% of the analyzed embryos/organs were mosaic. Over 96% of their aneuploid cells harbored ≤ 2 chromosome errors. Our findings have revealed a high prevalence of mosaicism in human embryos.

Associations Between Inter-Limb Asymmetry in Lower Limb Strength and Jump Performance in 14–15-Year-Old Basketball Players

This study aims to (1) assess the inter-limb asymmetry in hip, knee, and ankle strength and countermovement jump (CMJ) performance among adolescent basketball players and (2) examine the relationship between inter-limb asymmetry and CMJ performance. Moreover, 30 adolescent basketball players (15 boys and 15 girls) aged 14 to 15 years participated in this study. The strength of the lower limb joints was measured using an isokinetic dynamometer at a speed of 60 degrees per second. Three maximal CMJs were performed, and the highest jump was used for the final analysis. The subjects were median-split into high-asymmetry (n = 15) and low-asymmetry (n = 15) groups based on the calculated strength asymmetry scores. The asymmetry scores were calculated using the formula: (dominant–non-dominant)/dominant* 100%. The inter-limb asymmetry data ranged from 12.2% to 21.6%. A Spearman correlation analysis showed that only the inter-limb asymmetry of the ankle plantar flexor was significantly correlated with the CMJ heights (ρ = −0.56, p = 0.001). An independent t-test revealed no significant differences in strength asymmetry between boys and girls (all p > 0.05). The low-asymmetry group demonstrated significantly greater CMJ performance compared to the high-asymmetry group (ES = 1.11, 95% CI = 0.34–1.87, p = 0.007), indicating that inter-limb asymmetry of the ankle plantar flexor has a significant negative impact on CMJ performance. Coaches should focus on enhancing both the strength and symmetry of the ankle joints to improve athletic performance and prevent injuries in sports, where jumping is a common movement.
