Assessment of the quality of life in patients with cardiac arrhythmias using questionnaires is increasingly becoming the subject of clinical studies. However, there are no uniform criteria and standard norms for assessing the quality of life. Each questionnaire has its own criteria and assessment scale. This review presents the main general questionnaires that have been used in world practice to assess the quality of life in cardiology and arrhythmology. Special questionnaires are considered, which are used to clarify the parameters of the quality of life of interest in cardiac arrhythmias. Particular attention is paid to the assessment of this indicator in patients with ventricular arrhythmias.

EFFICIENCY OF NON-OPIOID ANALGESICS IN PATIENTS WITH TRAUMATIC LIMB AMPUTATIONS IN THE EARLY POSTOPERATIVE PERIOD

Justification : This work is the integration of safer and more effective drugs into clinical practice to optimize pain relief and reduce the risks associated with the use of traditional opioid analgesics. Objective: to evaluate the effectiveness of pain relief in the early postoperative period using non-opioid analgesics (ibuprofen, tafalgin, nefopam) in patients with mine-blast wounds (MBW) accompanied by traumatic amputations of the limbs. Methods : The study included 45 patients randomly assigned to three groups depending on the analgesic used: ibuprofen, tafalgin or nefopam. Results : In all groups, there was a significant decrease in the need for additional postoperative analgesia, including opioid drugs. Nefopam provided the most pronounced analgesic effect, reducing pain intensity on the VAS scale more than ibuprofen and tafalgin. However, its use was accompanied by a higher frequency of side effects (dizziness, tachycardia, hypertension). Ibuprofen demonstrated an optimal combination of analgesic efficacy and safety, with a minimal frequency of side effects. Conclusion: Nefopam has a pronounced analgesic effect in the early postoperative period in patients with MVR, but is associated with an increased frequency of side effects. Ibuprofen showed a high level of safety and sufficient efficacy, which makes it a preferred component of multimodal analgesia for this category of patients.

TRANSCRANIAL ELECTRIC PULSE THERAPY IN THE TREATMENT OF CHRONIC PAIN SYNDROME

Rationale : chronic pain syndrome in patients with combined mine-explosive injuries, traumatic limb amputations, extensive burns, etc., is currently especially relevant. The presented analysis of modern literature data on electrophysiotherapy of chronic pain in this category of patients. Objective : to study the literature data and conduct a comparative analysis of the currently used methods of transcranial electrophysiotherapy of pain syndrome, which will improve the results of treatment of patients with chronic pain syndrome. Materials and methods: the analysis of articles, reviews, and studies in the modern scientific medical literature on the voiced problem is carried out. The collection of materials was carried out in the electronic libraries of the kirov university, the first sechenov Moscow state medical university, articles from information resources were used: ncbi, pcm; springer; elsevier, embase, web of science and psychinfo, elibrary, etc. Results: a wide variety of transcranial electrophysiotherapy methods used in clinical practice in the treatment of chronic pain syndrome has been revealed in the literature. The methods are systematized based on: the nature of the intervention (invasive, non-invasive), the parameters of the electric current (direct, alternating, continuous or pulsed). Conclusion: methods of noninvasive transcranial electrophysiotherapy may be more effective and preferable for the treatment of chronic pain syndrome compared to other methods for a number of reasons: noninvasiveness, safety, simplicity, pain reduction due to regulation of neuroplasticity, regulation of emotional state.

Methods of anesthesiological support in the electrophysiological laboratory

The article presents modern methods of anesthetic aids used in the case of such interventional and surgical interventions as implantation of a pacemaker, cardiac resynchronization therapy, cardioverter defibrillator and catheter ablation. The advantages, disadvantages and problematic issues of anesthesia are discussed depending on the type of intervention and the patient’s condition. Based on the analyzed data, it is concluded that anesthesia during interventions in patients with arrhythmological profile is a global practice and emphasizes the positive impact of anesthesia methods on the quality and safety of procedures performed.
