Phase 3 Trial of Ibrutinib plus Rituximab in Waldenström’s Macroglobulinemia

BACKGROUND: Single-agent ibrutinib has shown substantial activity in patients with relapsed Waldenström's macroglobulinemia, a rare form of B-cell lymphoma. We evaluated the effect of adding ibrutinib to rituximab in patients with this disease, both in those who had not received previous treatment and in those with disease recurrence. METHODS: We randomly assigned 150 symptomatic patients to receive ibrutinib plus rituximab or placebo plus rituximab. The primary end point was progression-free survival, as assessed by an independent review committee. Key secondary end points were response rates, sustained hematologic improvement from baseline, and safety. The mutational status of MYD88 and CXCR4 was assessed in bone marrow samples. RESULTS: At 30 months, the progression-free survival rate was 82% with ibrutinib-rituximab versus 28% with placebo-rituximab (hazard ratio for progression or death, 0.20; P<0.001). The benefit in the ibrutinib-rituximab group over that in the placebo-rituximab group was independent of the MYD88 or CXCR4 genotype. The rate of major response was higher with ibrutinib-rituximab than with placebo-rituximab (72% vs. 32%, P<0.001). More patients had sustained increases in hemoglobin level with ibrutinib-rituximab than with placebo-rituximab (73% vs. 41%, P<0.001). The most common adverse events of any grade with ibrutinib-rituximab included infusion-related reactions, diarrhea, arthralgia, and nausea. Events of grade 3 or higher that occurred more frequently with ibrutinib-rituximab than with placebo-rituximab included atrial fibrillation (12% vs. 1%) and hypertension (13% vs. 4%); those that occurred less frequently included infusion reactions (1% vs. 16%) and any grade of IgM flare (8% vs. 47%). The major hemorrhage rate was the same in the two trial groups (4%). CONCLUSIONS: Among patients with Waldenström's macroglobulinemia, the use of ibrutinib-rituximab resulted in significantly higher rates of progression-free survival than the use of placebo-rituximab, both among those who had received no previous treatment and among those with disease recurrence. Atrial fibrillation and hypertension were more common with ibrutinib-rituximab, whereas infusion reactions and IgM flare were more common with placebo-rituximab. Rituximab combined with chemotherapy and interferon in follicular lymphoma patients: results of the GELA-GOELAMS FL2000 study

The FL2000 study was undertaken to evaluate the combination of the anti-CD20 monoclonal antibody rituximab with chemotherapy plus interferon in the first-line treatment of follicular lymphoma patients with a high tumor burden. Patients were randomly assigned to receive either 12 courses of the chemotherapy regimen CHVP (cyclophosphamide, adriamycin, etoposide, and prednisolone) plus interferon-alpha2a (CHVP+I arm) over 18 months or 6 courses of the same chemotherapy regimen combined with 6 infusions of 375 mg/m(2) rituximab and interferon for the same time period (R-CHVP+I arm). After a median follow-up of 5 years, event-free survival estimates were, respectively, 37% (95% confidence interval [CI], 29%-44%) and 53% (95% CI, 45%-60%) in the CHVP+I and R-CHVP+I arm (P = .001). Five-year overall survival estimates were not statistically different in the CHVP+I (79%; 95% CI, 72%-84%) and R-CHVP+I (84%; 95% CI, 78%-84%) arms. In a multivariate regression analysis, event-free survival was significantly influenced by both the Follicular Lymphoma International Prognostic Index score (hazard ratio = 2.08; 95% CI, 1.6%-2.8%) and the treatment arm (hazard ratio = 0.59; 95% CI, 0.44%-0.78%). With a 5-year follow-up, the combination of rituximab with CHVP+I provides superior disease control in follicular lymphoma patients despite a shorter duration of chemotherapy. Predictive factors of response and survival in myelodysplastic syndrome treated with erythropoietin and G-CSF: the GFM experience

We analyzed prognostic factors of response, response duration, and possible impact on survival of epoetin alpha, epoetin beta, or darbepoetin alpha (DAR) with or without granulocyte colony-stimulating factor in 403 myelodysplastic syndrome (MDS) patients. Sixty-two percent (40% major and 22% minor) and 50% erythroid responses were seen, and median response duration was 20 and 24 months according to IWG 2000 and 2006 criteria, respectively. Significantly higher response rates were observed with less than 10% blasts, low and int-1 International Prognostic Scoring System (IPSS), red blood cell transfusion independence, serum EPO level less than 200 IU/L, and, with IWG 2006 criteria only, shorter interval between diagnosis and treatment. Significantly longer response duration was associated with major response (IWG 2000 criteria), IPSS low to INT-1, blasts less than 5%, and absence of multilineage dysplasia. Minor responses according to IWG 2000 were reclassified as "nonresponders" or "responders" according to IWG 2006 criteria. However, among those IWG 2000 minor responders, response duration did not differ between IWG 2006 responders and nonresponders. Multivariate adjusted comparisons of survival between our cohort and the untreated MDS cohort used to design IPSS showed similar rate of progression to acute myeloid leukemia in both cohorts, but significantly better overall survival in our cohort, suggesting that epoetin or DAR treatment may have a favorable survival impact in MDS.

Prolonged PFS after Autologous Stem Cell Transplantation in Follicular Lymphoma Patients: A Long-Term Retrospective Study.

Abstract Background: The course of follicular lymphoma (FL) is characterized by iterative relapses. Several reports have demonstrated that intensive chemotherapy followed by autologous stem cell transplantation (auto-SCT) is an attractive treatment. Because a subgroup of patients obtains prolonged complete remission after auto-SCT, some authors have postulated that auto-SCT may cure FL patients. In order to address this issue and to characterize long-term responders, we retrospectively studied the outcome of 80 FL patients who underwent auto-SCT in our institution. Patients characteristics: Between 1989 and 2004, 80 FL patients underwent auto-SCT in the Haematology Department of Nantes Medical University, France. All patients had a FL according to the WHO classification. Median age at diagnosis was 45.7 (range 27–62) and median age at the time of transplantation was 48.1 (range 27–69). Auto-SCT was performed upfront in 36 cases, at first relapse in 36 cases and at a subsequent relapse in 8 cases. Median time from diagnosis to auto-SCT was 53 months. At diagnosis, 25 patients presented with a FLIPI &amp;lt;2 and ≥ 2 in 34 cases (data missing =21). Prior to auto-SCT, 71 patients received a CHOP or CHOP-like regimen and 24 patients received a cytarabine-containing regimen. The conditioning regimens prior auto-SCT were TBI-cyclophosphamide (63 patients) or BEAM (17 patients). The source of stem cell was either bone marrow (19 patients) or peripheral blood stem cell (59 patients); 2 patients received both. In note, the graft was purged using a CD34+ selection in 30 cases or a B-depletion in 19 cases. In the course of their disease (prior or after transplantation), 35 patients received Rituximab. Results: Median follow-up (calculated from the time of auto-SCT) was 6.2 years (range 0.2–16.6) (data missing =1). No patient died during the auto-SCT procedure. After auto-SCT, 71 patients reached CR/CRu. Thirty-five patients relapsed and 10 patients experienced histological transformation (HT)(including 8 cases with HT at first relapse). Myelodysplasia/AML occurred in 5 cases. At the time of the analysis, 68 patients were still alive. Causes of death were: HT in 6 cases, AML in 3 cases, follicular lymphoma related in 2 cases and not related to lymphoma in one case. The 3-, 5- and 7-year progression free survival (PFS) estimates were 74.7%, 66.5% and 58.6%, respectively. In note, a plateau occurred on the PFS Kaplan-Meier curve after 8 years. The 3-, 5- and 7-year overall survival (OS) estimates were 92.9%, 86% and 79.7%, respectively. In univariate analysis, five variables (age at the time of auto-SCT &amp;lt; 50y, a purged graft, a Rituximab-containing therapy, an extra-nodal excluding bone marrow involvement at diagnosis and FLIPI &amp;lt;2 ) were found to be statistically significant at a p&amp;lt;0.15 level and were included in the multivariate analysis. In multivariate analysis, the PFS was significantly better for patients with a FLIPI&amp;lt;2 (p=0.043) and patients&amp;lt;50 years at the time of auto-SCT (p=0.03). A trend for a better PFS was observed for patients who received Rituximab in the course of the disease (p= 0.084). Conclusion: By multivariate analysis, we demonstrate that the FLIPI is a strong predictive index to predict PFS after auto-SCT. Moreover, our results confirm the major interest of auto-SCT in the treatment of FL and highlight that the opportunity to perform auto-SCT should not be jeopardize during the course of the disease. Finally, the plateau on the PFS curve suggests that some FL patients could be cured by auto-STC.

A randomized trial of hydroxyurea versus VP16 in adult chronic myelomonocytic leukemia. Groupe Francais des Myelodysplasies and European CMML Group

We performed a randomized study of hydroxyurea (HY) versus VP16 in advanced chronic myelomonocytic leukemia (CMML) patients with CMML (according to French-American-British group criteria) and either documented visceral involvement (excluding liver and spleen infiltration) or at least 2 of the following: (1) neutrophils > 16 x 10(9)/I (2) Hemoglobin < 10 g/dL (3) platelets < 100 x 10(9)/L (4) marrow blasts > 5% (5) spleen > 5 cm below costal margin were eligible for this trial. Initial dosage was 1 g/d for HY and 150 mg/week for VP16, orally (doubled in case of visceral involvement). Doses were scheduled to be escalated up to HY 4 g/d and VP16 600 mg/week in the absence of response, and finally adjusted to maintain white blood cells (WBCs) between 5 and 10 x 10(9)/L. Crossing over was scheduled only in case of life threatening visceral involvement or major progression. The major endpoint of the study was survival. The study was closed on first interim analysis that showed a superiority of HY over VP16, after inclusion of 105 pts (HY arm: 53, VP16 arm: 52). Results of the second interim analysis, performed 7 months later, are presented here. Median age was 71 (range 38 to 91), median WBC count 20.10(9)/L (range 10 to 187). Thirteen pts had visceral involvement (3 serous effusions, 8 cutaneous infiltrations, 1 kidney, 1 bone infiltrations). Initial characteristics were similar in the HY and VP16 groups. Median follow up was 11 months in both groups (range 1 to 43+). Response to treatment was seen in 60% of the pts in the HY group, versus 36%, respectively, in the VP16 group (P = .02). Time to response was significantly shorter in the HY group (2.1 v 3.5 months, in the VP16 group, P = .003) and response duration was significantly longer in the HY group (median 24 v 9 months, in the VP16 group, P = .0004). The response rate of patients with visceral involvement was 3 out of 7 in the VP16 arm versus 5 out of 6 in the HY group. Three of the 10 pts crossed over from HY to VP16 responded as compared to 6 pts of the 11 pts crossed over from VP16 to HY. HY yielded better response on leukocytosis (P = .002). The effect on splenomegaly platelets, on hemoglobin level and transfusion requirement was similar in the 2 treatment groups. A significantly higher incidence of alopecia was noted in the VP16 arm (20% v 3%, P = .03). Fourteen (27%) and 20 (38%) patients in the HY and the VP16 group respectively, progressed to acute myeloid leukemia (difference NS). Twenty five (53%) and 44 (83%) patients in the HY and the VP16 group, respectively, had died (P = .002). Median actuarial survival was 20 months in the HY arm, versus 9 months in the VP16 arm (P < 10(-4)). Main factors associated with poor survival were allocation to the VP16 arm, "unfavorable" karyotype (ie, monosomy 7 or complex abnormalities) and anemia. In the HY group, unfavorable karyotype (P = .006), and low hemoglobin level (P = .004) were significantly associated with low response rates. Prognostic factors for poor survival in the HY group were also unfavorable karyotype (P = .001), and low hemoglobin level (P < 10(-4). In conclusion, we found that HY gave higher response rates and better survival than VP16 in advanced CMML. However, even with HY responses were only partial and survival was generally poor. This stresses the need for new agents in the treatment of CMML, that will have to be compared with HY in future randomized studies.
